Clairmont Place Senior Living Community is Hiring a Director of Clinical Services Near Decatur, GA

Description

The Montclair is a Assisted Living Home in Decatur, Ga, part of the Clairmont Place Community. We are tucked away on 16 acres of land with a private lake and walking paths, in a park-like setting. We are conveniently located off Clairmont Rd, near Emory and the VA Hospital. The Montclair is owned by the Clairmont Place Community members and run under the direction of a board of resident representatives.

The Montclair offers a continued, higher level of support for our elders who are no longer able to live independently. Our focus and goal is person-centered care with a high level of engagement and a low resident to caregiver ratio, allowing our caregivers to focus and individualize their attention on elders and their care plans. Our management staff has a combined 30 years of leadership experience. Montclair's team of care partners help residents perform tasks safely, prioritizing autonomy, engagement and independence. We care for the human spirit as well as the body.

The right person for this role will have the ability to focus individual care without sacrificing their vision for task completion for the day, the ability to work with a team to accomplish goals, the ability to communicate with care, consideration and professionalism with both elders, teammates and family members, and the ability to serve as a true partner in caregiving for our residents. You must have experience working with elders and heart for serving this population. Ideal candidate must have managerial and leadership experience. 

This position requires LPN license, 5 years' experience. Clairmont Place requires new hires to be vaccinated against Covid - we serve a vulnerable population. New hires must be able to pass a drug and background screen, a physical and have a negative TB test. You must have current CPR certification.

This is a full-time salary position, average of 40 to 50 hours per week. This candidate must be flexible to work weekend, holidays. Benefits include health care through Aetna, vision and dental coverage through MET Life, vacation time, sick leave, personal time, paid holidays, paid short term disability, paid life insurance and 401K matching. We also offer two great Employee Assistance programs which offer therapy sessions, financial and legal advice, and other support.

Essential Functions:

  • In consultation with the Administrator, trains, and disciplines clinical employees in accordance with Montclair Assisted Living at Clairmont Place human resources policy.
  • Monitors day-to-day performance of staff as it relates to general work duties and responds appropriately to both strengths and opportunity outcomes.
  • Seeks opportunities to maintain education, knowledge and skills.
  • Monitors the third-party clinical providers working at the community.
  • Assesses prospective residents for appropriateness of residency.
  • Coordinates New Resident Orientation processes to ensure a smooth transition.
  • Manage clinical staff scheduling.
  • Ensures the highest level of resident care is completed with routine visits with residents, family, daily walk-throughs, periodic audits/surveys and timely follow up with concerns.
  • Coordinates with physicians to ensure high quality of care for residents with service plans, orders and care delivery.
  • Complete service plans in accordance with regulatory guidelines.
  • Conducts family conferences to determine and educate regarding the amount of support services required to meet the residents' needs.
  • Responds to emergencies to assess situations and alert medical help when necessary.
  • Provide clinical staff support in emergencies and non-emergencies.
  • Addresses and follows up on all resident concerns, incidents, and accident reports in a timely manner per policy.
  • Contacts family members during times of emergency and for other issues of concern.
  • Communicates on a regular basis with health care providers to monitor and assess health conditions, safety issues, and the needs of all residents.
  • Communicates with other staff members concerning resident changes and building operation issues.
  • Manages labor and supplies budget within guidelines.
  • Maintains confidentiality of business policies and procedures as well as clinical and other information and with residents and potential residents’ information.
  • Other duties as assigned.

Non-Essential Functions:

  • Participates in Manager On-Call program.
  • Attends all required training, in-service, and staff meetings.
  • Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
  • Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
  • Adheres to all policies and procedures of Montclair Assisted Living at Clairmont Place.
  • Performs other duties as assigned by the Administrator.

Requirements

QUALIFICATIONS/SKILLS/REQUIREMENTS

  • Holds an active, unencumbered Georgia licensed professional nursing license in the state in which the community is located.
  • Must have the ability to remain calm in stressful situations, to be flexible, to work well with many interruptions and have skill in multi-tasking.
  • Ability to utilize computers, software and other technology.
  • Well versed in the Department of Community Health Rules and Regulations for Assisted Livings.
  • Experience as a Clinical Director in an Assisted Living community.
  • 2 years’ experience preferred.
